Sick Notes

If you need to take time off from work because you are ill, there are different types of sick notes:
Self Certificate
If you have been off for up to 7 days you can self-certify. Self-certification forms can be obtained from your employer, or
downloaded from HMRC and completed yourself. Your employer should accept this from you. If they refuse to do this there are two options, either ask them to write to us for a Private sick note and we will charge them directly for this or request a Private Sick note yourself for which you will be charged.
Med 3
If you have been off for more than 7 days and require a sick note you will need to see a Doctor and ask for Med 3.
Med 4
If you have been off sick for a long time then the DWP may request a Med 4, which is a more detailed sick note. A Doctors appointment will usually be required for this.
Med 5
If you have been in hospital or require a back dated sick note a Med 5 should be requested via reception
